Moving to Adult Care

Policy:

Stony Brook Pediatrics transitions patients to adult care when that patient has completed undergraduate education, specific vocational training, or has completed high school with the intention to join the community’s workforce. It is also our policy to transition a patient to adult care if the patient becomes a parent.

Procedure:

  • A paper copy of the patient’s medical summary from our electronic medical record will be given to the patient at the time of transfer. This is done preferably in the office during a dedicated final office encounter.
  • A prepared list of local providers will be given to the patient to assist in selecting a new healthcare provider.
  • Individual considerations factoring into the transition of care, and any special medical needs, will be identified and discussed with the patient and documented in the medical record.
  • A release of records form will be signed by the patient, and scanned into the patient records. The patient’s medical record will then be sent directly to the future provider.
